This presentation describes the Jackson Health System’s new naming convention process for patients who are “unknown.” It also identifies the acceptable criteria sources to confirm patient’s identity, identifies the timing and Medical Information Record Advancing Clinical Excellence (MIRACLE) process for updating name changes of previous unidentified patients, and describes the identity confirmation process for behavioral health employees.
