Dr. Lee shared how social determinants of health were addressed at when the integrated care team determined that clients with COVID-19 symptoms were unable to secure transportation for testing at the health department. The FQHC partnered with the local health department to establish a testing site at the clinic, which was open for all residents in the area. This ensured that the clinic could take care of their clients and also serve the community at large.
